Important Insurance Reminder
Because insurance networks vary by plan — and not all plans within a network are covered — we recommend that you confirm with your insurance company as well that Bhalla Endocrinology is in-network with your specific plan.
Patients are responsible for copays, coinsurance, and deductibles as required by their insurance plan. All patient financial responsibility is due at the time of service, prior to being seen. If your deductible has not yet been met, the amount due will be collected before the visit.
Referrals
If your insurance plan requires a referral from your primary care physician (for example, HMO plans), please obtain this before scheduling your new patient appointment. Referrals are typically issued for a limited number of visits or for a set time period, so patients are responsible for ensuring their referral remains current for follow-up appointments.
